CLIENT INFORMATION

⚠️ NOTICE TO CONSUMER (Required by California Business and Professions Code Section 6408)

I AM NOT AN ATTORNEY and PHN Legal Document Assistance is not a law firm. I am a registered Legal Document Assistant and I AM AUTHORIZED TO PREPARE LEGAL DOCUMENTS ONLY UNDER YOUR SPECIFIC DIRECTION.

I CANNOT:

  • Give you legal advice
  • Represent you in court or before any government agency
  • Select legal forms for you
  • Tell you what your legal rights are
  • Advise you on what answer or information you should provide on forms
  • Advise you on the likely outcome of your case
  • Make strategic legal decisions on your behalf

IF YOU NEED LEGAL ADVICE, YOU SHOULD CONSULT WITH AN ATTORNEY.

Bond Amount: $25,000.00

I am bonded and registered in Los Angeles County as required by California Business and Professions Code Section 6400, et seq.

SCOPE OF SERVICES

PHN Legal Document Assistance agrees to provide the following self-help services, performed only under the Client's specific direction and instruction:

SERVICES NOT PROVIDED - IMPORTANT LIMITATIONS

As a Legal Document Assistant, PHN Legal Document Assistance is prohibited by law from providing the following services:

  • Providing legal advice, opinions, or recommendations
  • Representing you in court, at hearings, or in any legal proceedings
  • Selecting which legal forms you should use
  • Advising you on what information to provide on forms or documents
  • Advising you on court procedures, deadlines, or legal strategies
  • Interpreting laws, court orders, or legal documents for you
  • Appearing on your behalf before any court or government agency
  • Making any legal decisions on your behalf
  • Negotiating settlements or agreements on your behalf

IMPORTANT: You are solely responsible for the content, legal sufficiency, and accuracy of all information you provide. You are responsible for all legal decisions and strategies in your case. If you need legal advice, you must consult with a licensed attorney.

FEES AND PAYMENT

The Client agrees to pay for services as follows:

Additional Costs (NOT included in fees above): Court filing fees, process server fees, courier fees, certified copy fees, and other third-party costs are NOT included in the service fee. These costs will be billed separately or paid directly by Client.

Accepted Payment Methods:

ITEMIZED STATEMENT REQUIREMENT

Before providing any services, PHN Legal Document Assistance will provide you with a written itemized statement showing the fee for each specific service to be performed. You have the right to receive this itemized statement before agreeing to any services.

TURNAROUND TIME

Standard turnaround times for services are as follows:

  • Simple forms (1-3 pages): 3-5 business days
  • Complex forms (4-10 pages): 5-7 business days
  • Document packages (multiple forms): 7-10 business days
  • Court filings: 2-3 business days after document completion
  • Custom timelines: As agreed in writing

Turnaround time begins when:

  • Client provides ALL required information and documents
  • Payment is received (or deposit for payment plans)
  • PHN Legal Document Assistance confirms receipt and acceptance of the project

Delays caused by incomplete information, client revisions, or late payments will extend the turnaround time accordingly.

AVAILABILITY AND BUSINESS HOURS

PHN Legal Document Assistance standard business hours are:

  • Monday - Friday: 9:00 AM - 5:00 PM Pacific Time
  • Saturday: By appointment only
  • Sunday: Closed
  • Holidays: Closed

Clients may contact PHN Legal during business hours via:

Response times:

  • Phone calls during business hours: Same day
  • Emails: Within 24-48 business hours
  • After-hours messages: Responded to next business day

EXPEDITED AND SAME-DAY SERVICES

PHN Legal Document Assistance offers expedited services for urgent matters at additional cost:

Same-Day Service (completion within 24 hours):

  • Additional fee: 100% of standard service fee
  • Must be requested before 10:00 AM Pacific Time
  • Subject to availability - not guaranteed
  • Payment in full required before work begins

Rush Service (completion within 48-72 hours):

  • Additional fee: 50% of standard service fee
  • Subject to availability
  • Payment in full required before work begins

After-Hours/Weekend Service:

  • Additional fee: $75/hour surcharge plus standard rates
  • By appointment only
  • Must be scheduled at least 24 hours in advance

Client acknowledges that expedited timelines do not guarantee court acceptance or legal sufficiency of documents.

EMERGENCY AND URGENT MATTER POLICY

PHN Legal Document Assistance will make reasonable efforts to accommodate true emergencies but cannot guarantee availability.

True emergencies include:

  • Court deadline within 2 business days
  • Restraining order filings
  • Response to eviction with imminent court date
  • Other time-sensitive legal deadlines

To request emergency services:

  • TEXT (213) 557-0930 - available 24 hours for emergencies
  • Explain the nature of the emergency and deadline
  • Provide ALL required information and documents immediately
  • Pay emergency service fee in full before work begins

REFUND POLICY

If Client terminates this agreement or requests a refund:

  • Fees for services already completed are non-refundable
  • Client will receive a refund for any fees paid for services not yet performed
  • A $50 administrative fee may be deducted from refunds
  • Any third-party costs already incurred on Client's behalf will be deducted
  • Refund requests must be made in writing to PHN Legal Document Assistance
  • Refunds will be processed within 30 days of written request

CLIENT RESPONSIBILITIES

The Client agrees to:

  • Provide complete, accurate, and truthful information
  • Make all legal decisions regarding the case
  • Carefully review all documents before signing or filing
  • Appear in court if required (PHN Legal Document Assistance cannot appear for you)
  • Pay all fees and costs according to the agreed payment schedule
  • Provide all necessary documents and information in a timely manner
  • Understand that I am responsible for my own legal matter
  • Consult with an attorney if I need legal advice

CONFIDENTIALITY

PHN Legal Document Assistance will keep all information provided by Client confidential and will not disclose it to third parties except:

  • As required by law or court order
  • With Client's express written permission
  • To third parties necessary to complete services (e.g., process servers, court personnel)
  • To defend against legal claims made against PHN Legal Document Assistance

NO ATTORNEY-CLIENT RELATIONSHIP OR PRIVILEGE

This agreement does NOT create an attorney-client relationship. PHN Legal Document Assistance is NOT a law firm and cannot provide legal representation or services that only licensed attorneys are authorized to provide. Communications with PHN Legal Document Assistance are NOT protected by attorney-client privilege.

TERMINATION

Either party may terminate this agreement at any time by providing written notice. Upon termination:

  • Client will pay for all services performed up to the termination date
  • PHN Legal Document Assistance will refund any fees paid for services not performed (see Refund Policy)
  • PHN Legal Document Assistance will return all original documents within 10 business days
  • PHN Legal Document Assistance will provide a final accounting of all fees and costs

RIGHT TO REFUSE SERVICE

PHN Legal Document Assistance reserves the right to refuse service or terminate this agreement at any time for reasons including but not limited to:

  • Client provides false, incomplete, or misleading information
  • Client requests services outside the scope of Legal Document Assistant practice
  • Client requests services that PHN Legal Document Assistance is legally prohibited from providing
  • Client is abusive, threatening, or harassing to PHN Legal Document Assistance staff
  • Client fails to pay fees or costs as agreed
  • Conflict of interest exists or arises during representation
  • Client's matter requires legal advice or representation by an attorney

DISCLAIMER OF WARRANTIES AND GUARANTEES

PHN Legal Document Assistance makes NO warranties or guarantees about:

  • The legal sufficiency or correctness of any documents prepared
  • The outcome of any legal matter or court case
  • Whether documents will be accepted by the court
  • The legal effect or consequences of any documents
  • Court procedures, deadlines, or requirements

Clients are solely responsible for determining whether documents meet their legal needs and for all legal strategies and decisions. Clients should consult with an attorney if they have questions about legal sufficiency or strategy.

LIMITATION OF LIABILITY

PHN Legal Document Assistance total liability under this agreement shall not exceed the total fees paid by Client for services. PHN Legal Document Assistance is not liable for any indirect, consequential, or punitive damages. PHN Legal Document Assistance is not liable for court decisions, legal outcomes, or consequences resulting from Client's legal decisions or information provided by Client.

BONDING AND REGISTRATION INFORMATION

PHN Legal Document Assistance maintains a $25,000 surety bond as required by California Business and Professions Code Section 6402. A copy of the bond and registration certificate is available upon request.

COMPLAINT AND DISPUTE RESOLUTION

If you have a complaint about services provided by PHN Legal Document Assistance, you may:

  • Discuss your concerns directly with PHN Legal at (213) 557-0930
  • File a written complaint with PHN Legal at the address listed below
  • File a complaint with the Los Angeles County Clerk's Office
  • Consult with an attorney regarding your legal options
  • File a complaint with the California Department of Consumer Affairs
  • Make a claim against the surety bond

GOVERNING LAW AND VENUE

This agreement shall be governed by and construed in accordance with the laws of the State of California. Any disputes arising from this agreement shall be resolved in Los Angeles County, California. This agreement complies with California Business and Professions Code Sections 6400-6415.

ENTIRE AGREEMENT

This agreement constitutes the entire agreement between the parties and supersedes all prior agreements or understandings. This agreement may only be modified in writing signed by both parties.

ELECTRONIC SIGNATURE CONSENT

By signing this agreement electronically, both parties agree that electronic signatures are legally binding and have the same effect as handwritten signatures. Client has the right to request a paper copy of this agreement at any time.
